I have knowledge and consent of your landlord, or the person owning the premises in which the animal will live, and all household members, to foster animals in my home. I understand that Animals for Life reserves the right to verify all information submitted on this application, including veterinary information. I understand it is my responsibility to provide a safe, warm, indoor housing for fostered animals(s). I understand that despite best efforts, some animals may not survive due to physical or other health-related complications. I understand that fostered cats/kittens are not allowed outdoors at any time. I understand that I must spend as much time as possible socializing and playing with the foster animal(s). I understand that I must closely monitor the condition of the fostered animal(s) and contact Animals for Life immediately when problems are observed or when I have questions/concerns. I understand that all veterinary care must be authorized in advance by Animals for Life. If an emergency arises, I must contact an Animals for Life representative or go directly to the emergency vet approved by Animals for Life. I agree to comply with the Animals for Life rules and regulations, instructions and mandates, and understand that failure to comply may result in immediate termination as a Foster family. I understand that my services are provided strictly as a volunteer basis without any pay or compensation of any kind and without liability of any nature on behalf of Animals for Life; all services to be performed are at my own risk. I understand and acknowledge that I do not have any right or authority to keep, adopt, transfer, or place foster animal(s) in other homes or with other individuals. I agree that every animal I provide foster care for must be physically returned to Animals for Life by the date set forth below or at any time upon request of Animals for Life. I also agree to return the animal(s) immediately if I am no longer able to provide adequate care. I understand that medicines and other supplies provided by Animals for Life are for use with foster care animals only, and are not to be administered to animals that are not the property of Animals for Life. I understand that in handling animals there exists a risk of injury, infections, disease including physical harm caused by animals to me, my family, visitors in my home, and my pets. On behalf of myself, my heirs, personal representatives and executors, I hereby release, discharge, indemnify, and hold harmless Animals for Life, its agents, servants, and board members from any and all claims, causes of actions, or demands and any nature of cause, including costs and attorney fees incurred or sustained by me in any way connected with my services to Animals for Life but not limited to: animal bites, accidents, injuries, property damage and veterinary fees. I understand that the foster animal(s) are temporarily in my care and belong exclusively to Animals for Life. I also understand that the purpose of this foster application is solely to provide care for the foster animal(s).
